Bellamy hit his face with his Seven String Manson guitar during the beginning of Citizen Erased. He tried to continue, but had to stop and be taken to hospital. Someone recorded and uploaded it to Youtube, and the video can be watched here.

There are conflicting - or rather ambigious - testimonies regarding what number on the set Citizen Erased was, however it appears that it was 5th.[2]
